How to Fill Visa Application Form to Apply Schengen Visa

Everyone who are going apply Schengen Visa to Visit Europe Countries, is needed to fill in and sign the visa application form. The visa form also requires the applicant to attach a photograph taken consistent with the visa photograph specifications and requirements.

13. Travel Document Number

14. Issue Date

Write the date your passport was issued (Please don’t put the extension date).

15. Expiration Date

Write the date as indicated in your passport, taking under consideration any extension.

16. Issued By:

Specify the authority that issued the passport and therefore the place of issue.

17. Applicant’s home address, email address and phone number

Write the address specifying whether it’s your personal mailbox or that of another person or company (in this case, write the name of the mailbox owner).

25. Length of Stay

27. Fingerprints

32. Host Firm or Organization Name and Address

Write the name and address of the firm or organization that invited you (telephone and fax) and therefore the contact information for your contact person within the firm.

33. Living and Travel Expenses while You’re Stay Are Covered by?

Check the acceptable box (you may check several boxes).

34. Personal Information regarding a loved one who may be a European Union, EEA, or Swiss National

Write the Full name, nationality, date of birth, passport number, and relationship as long as you’re the dependent spouse, child, father, or mother of an individual who may be a Europe Union citizen.

35. Relationship

Check the acceptable box if you’re a loved one of a Europe Union, EEA, or Swiss national.

36. Date and site

37. Signature

The Visa form should be completely filled and signed by Visa applicant and not another person. If the visa applicant may be a minor (under 18), the appliance must be signed by an individual having parental authority and their relationship should be included in parentheses (father, mother, and guardian). If the visa applicant cannot sign, a cross could also be used.
